CANDIDATE PRE-CONSULTATION QUESTIONNAIRE 

This questionnaire is intended for individuals who are considering uniting to Lucifera through the Temple of Nyx, including those seeking an Acknowledgement to Lucifera, spiritual connection through the Temple, or consideration for the Path of Lucifera. 

The purpose of this questionnaire is to help determine whether the Temple and the candidate are spiritually, personally, and practically compatible before proceeding with a formal consultation. 

Please answer every question completely and honestly. There are no “perfect” answers. The purpose is to understand your current path, intentions, experience, commitments, and readiness. 

Please explain why you have chosen to contact the Temple of Nyx at this time. What specifically attracted you to the Temple? 

What are you hoping to learn, experience, receive, or become through your connection with the Temple? 

Was there a particular teaching, experience, spiritual event, personal realization, companion, dream, calling, or other circumstance that caused you to seek the Temple?

Why do you wish to formally acknowledge Lucifera? 

Are you prepared to respect the rules, boundaries, policies, and spiritual order of the Temple while participating as an acknowledged member? 

Do you understand that the Acknowledgement is not the Path of Lucifera and does not constitute Black Salvation? 

IF YOU ARE SEEKING THE PATH OF LUCIFERA: 

Why do you desire the Path of Lucifera? 

Are you prepared for a continuing devotional path involving learning, discipline, prayer, consultation, spiritual work, service, courses, rites, and personal development? 

Are you prepared to devote the necessary time to your Path rather than treating it as a temporary interest? 

Are you prepared to receive guidance from a Sire chosen by Lucifera if you are accepted?

By submitting this questionnaire, I affirm that the information I have provided is truthful and complete to the best of my knowledge. 

I understand that submitting this questionnaire does not guarantee acceptance, membership, approval for a binding or companion, admission to the Path of Lucifera, or eligibility for any particular rite, service, or spiritual process. 

I understand that the Temple of Nyx may request additional information or clarification before determining whether a consultation should proceed. 

I understand that I am responsible for asking questions about anything I do not understand before voluntarily agreeing to any Temple commitment, rite, policy, or spiritual practice. 

I further acknowledge that my participation is undertaken by my own free will and that I may decline to proceed before making a formal commitment.
